Title

RESOLUTION of the City of San Leandro City Council to Approve a Consulting Services Agreement with Ethosoft, Inc. for the Installation and Configuration of a Laboratory Information Management System for an Amount not to Exceed $54,305; to Authorize Individual Agreement Amendments up to 10% ($5,430); and to Approve Appropriation of Funds from the Water Pollution Control Plant Fund Balance Reserves of $43,065

 

Body

WHEREAS, an agreement between the City of San Leandro and Ethosoft, Inc., a copy of which is attached hereto, was presented to this City Council; and

 

WHEREAS, the City Council is familiar with the contents thereof; and

 

WHEREAS, the City Manager recommends approval of said agreement.

 

NOW, THEREFORE, the City Council of the City of San Leandro does RESOLVE as follows:

 

That the City Council hereby authorizes the City Manager to accept the proposal herein mentioned; negotiate, approve, and execute the agreement for the installation and configuration of a Laboratory Information Management System and first year of service with Ethosoft, Inc. for an amount not to exceed $54,305; and

 

That the City Manager or his designee is authorized to negotiate and approve agreement amendments to the consulting services agreement up to 10% (or $5,430) of the original contract amount; and

 

That the City Manager is authorized to make non-substantial revisions to said agreement, subject to approval as to form by the City Attorney; and

 

The approved appropriation of funds for $43,065 from the Water Pollution Control Plant Fund Balance reserves to account 593-51-002-5120.
